It is extremely important to document all incidents as soon as possible. Even if you believe you were in no way responsible for any injury or damage, the incident should be recorded and preserved.

The incident report should contain:
Completed Incident Report Form (click here to download)
• Witness contact information
• Statements made by witnesses
• Rental agreement (original or copies)
• Maintenance and Inspection Records on the unit involved (original or copies)
• Photographs taken following the incident

If there is any indication that a claim may arise from the incident, you should immediately file a claim report with the program administrator and insurance carrier.
